University of Waterloo verification of condition forms are available for students who are unable to fulfil academic obligations on a short-term, temporary basis.
What you need to know
Campus Wellness Process
- Visit University of Waterloo Campus Wellness for an assessment of the condition, to receive appropriate medical care, to receive an accurate diagnosis and to receive a Verification of Condition (VOC) Form that will be completed by your health care practitioner.
- Campus Wellness will complete and upload the VOC Form for you to the Absence Database for your eligibility for academic consideration to be assessed.
- Your instructors will be automatically notified of your submitted VOC Form. Contact them promptly to discuss academic considerations for missed work, as the final decision is theirs.
Please be aware that your Campus Wellness health care practitioner may charge a $20.00 fee for completion of the VOC Form.
Off-campus Practitioner Process

Visit your family health care practitioner during the condition to receive appropriate medical care, and an accurate diagnosis.
- Bring the University of Waterloo Verification of Condition (VOC) Form (PDF) with you to be completed by your health care practitioner.
- Upload the completed VOC Form, with all signatures, to the Absence Database for your eligibility for academic consideration to be assessed.
- Refer to the Absence Database for next steps.
There is no need for students who have seen a health care practitioner off-campus to have their VOC Form verified by an on-campus health care practitioner.
