Use this form to allow someone else to pick up your official transcript, proof of enrolment letter, or duplicate/replacement diploma. You must submit your official document request prior to providing authorization. Ensure that you select the Pick up option when ordering your document.

Please wait until you have received a confirmation email that the document is ready before having the authorized person come in, except in cases where you are designating someone to pick up a diploma that you did not collect at convocation.
