Submission of this form confirms that you are choosing to receive a Magisteriate designation on your diploma instead of the Master's degree.
Important information
- The term "Magisteriate" will only appear on your diploma. Transcripts and any degree verification will only refer to "Master". This terminology discrepancy between documents may result in border-crossing issues.
- A Request for a Replacement Diploma Form with associated fees will be required if you submit after the deadline.
