Students who have received an official OGS/QEII-GSST offer from the University of Waterloo should use this form to:
- Accept or decline an offered OGS or QEII-GSST.
Submit this form no later than three weeks after receipt of your award offer. Refer to your official award offer email for the specific deadline and award offer details.
