Ontario Graduate Scholarship (OGS)/Queen Elizabeth Scholarship in Science & Technology (QEII-GSST) accept/decline form

Students who have received an official OGS/QEII-GSST offer from the University of Waterloo should use this form to:

• Accept or decline an offered OGS or QEII-GSST.
  • Submit this form by the deadline indicated in your official award offer email.

Newly admitted students: To log into this form, you must first set up your Waterloo Identity, if you have not already done so. To do this, locate your Waterloo ID in your admission offer letter and complete the Claim your Waterloo identity process. Once your identity has been created, use it to log into the OGS/QEII-GSST accept/decline form by adding @uwaterloo to the end of your WatIAM ID.
