Complete this form to request a transfer from Laurier's BBA/BMath or BBA/BCS program to a single degree program within the Faculty of Mathematics at the University of Waterloo, only if:

• You meet all of the admission requirements.
• Your home institution is Wilfrid Laurier University. University of Waterloo students must submit a Plan Modification Form.

Application deadlines

Complete this form at least one month prior to the start of next term.

• December 1 (for winter term admission)
• April 1 (for spring term admission)
• August 1 (for fall term admission)
