Post-admission requirements

Notice for all PharmD students

Due to recent Ontario College of Pharmacists (OCP) regulation changes (effective October 1, 2024) and updates to the School's requirement verification processes, the following post-admission requirements are broken down into cohort specific information. Please refer to the information outlined below for your current cohort.

To participate in and complete the experiential curricular components of the PharmD program, students must fulfill a number of immunization and documentation requirements. Expenses related to completion of these requirements are the responsibility of the student.

Students who fail to comply with immunization and documentation requirements may result in cancellation of co-op placements, patient care rotations, withholding of academic grades and/or denial of academic progression. Any student who requires an accommodation (due to disability, illness or condition) should contact AccessAbility Services as well as the School's Pharmacy Advisor team.

If a requirement expires during the program, students should renew the requirement (before it expires) and maintain a record in case the School or practice site requires proof of completion. This is common for some vaccinations (e.g. Tdap), certifications (e.g. first aid/CPR), as well as professional liability insurance policies. The two exceptions are TB skin tests and vulnerable sector checks (VSC). Students will only need to complete another one-step TB skin test or VSC if it is a co-op site requirement and your most recent test/check has expired or is not within the site-specific timelines. Students do not need to resubmit anything to Verified between their year 1 and year 4 Electronic Record Verification Reviews. Their Verified status will likely move to "expired" during this time.

Rx2026

First year (pre-work term) requirements:

Due date: April 1, 2025 (prior to work term 3 in Spring 2025)

Where to submit: Upload documentation in the appropriate folder of your requirements in CORE ELMS.


Fourth year (pre-patient care rotation) requirements:

Due date: December 1, 2025

Where to submit:

  • Verified by Synergy Gateway (access will be provided in Fall 2025).
  • Note: Some requirements (e.g. résumé, student profile, and site-specific requirements should be uploaded to the appropriate folder of your requirements in CORE ELMS.

Rx2027

First year (pre-work term) requirements:

Due date: July 1, 2024

Due date: December 1, 2024

Where to submit:  Upload documentation to the appropriate folder of your “Requirements” in CORE ELMS.


Fourth year (pre-patient care rotation) requirements:

Due date: December 1, 2026

Where to submit:

  • Verified by Synergy Gateway (access will be provided in Fall 2026).
  • Note: Some requirements (e.g. résumé, student profile, and site-specific requirements should be uploaded to the appropriate folder of your requirements in CORE ELMS.

Rx2028

First year (pre-work term) requirements:

Due date: February 1, 2025

Where to submit: Verified by Synergy Gateway (access provided early Dec 2024).


Fourth year (pre-patient care rotation) requirements:

Due date: December 1, 2027

Where to submit:

  • Verified by Synergy Gateway (access will be provided in Fall 2027).
  • Note: Some requirements (e.g. résumé, student profile, and site-specific requirements should be uploaded to the appropriate folder of your requirements in CORE ELMS.

Rx2029

First year (pre-work term) requirements:

Due date: December 1, 2025

Where to submit: Verified by Synergy Gateway (access provided late summer/early fall 2025).


Fourth year (pre-patient care rotation) requirements:

Due date: December 1, 2028

Where to submit:

  • Verified by Synergy Gateway (access will be provided in Fall 2028).
  • Note: Some requirements (e.g. résumé, student profile, and site-specific requirements should be uploaded to the appropriate folder of your requirements in CORE ELMS.
