Use this form to:
- Order transcripts for the CGRS D (Canadian Institutes of Health Research (CIHR), Natural Sciences and Engineering Research Council (NSERC) and Social Sciences and Humanities Research Council (SSHRC) Doctoral Scholarship competitions through Waterloo. The submission deadline for this form is Monday, September 8, 2025.
Transcripts are required for all programs of study listed in the application (CIHR – CIHR CV, NSERC – Academic Background, SSHRC – SSHRC CV), including if you are just starting your program in fall 2025. Additionally, if you were on exchange for any terms and the courses do not have an actual grade showing on your home university transcript, you will require official transcripts from the exchange institution.
- GSPA will fulfill transcript requests by generating official Waterloo transcripts ONLY. Waterloo transcripts will be emailed to the student by GSPA.
Students are responsible for obtaining all other official transcripts from previous institutions, including transcripts from institutions where they completed an exchange term. Transcripts must be uploaded to the application portal for any institution identified on the application.
