HLTH432 Honours Thesis enrollment form

This page contains the HLTH432 Honours Thesis Enrollment Form. The form will be used as an Override to Enroll the approved students into HLTH432. It also acts as the Department Consent Form and allows students to indicate their intended/approved usage of the course. Please note that you may only submit this form AFTER you have received approval from the School of Public Health Sciences (SPHS) Associate Director as well as your thesis supervisor.  

The HLTH432 Information Guide contains all the information you need to know with regards to the steps of enrolling into HLTH432. Please read it carefully prior to you submit this form.

This form needs to be submitted no later than one week prior to the last day to add courses.
