This page contains the HLTH474 Health Apprenticeship Enrollment Form. The form will be used as an Override to Enroll the approved students into HLTH474. Please note that you may only submit this form AFTER you have received approval from your supervisor.  

This form needs to be submitted no later than one week prior to the last day to add courses.
