GERON401A/B enrollment form

This page contains the GERON401A/B Enrollment Form. The form will be used as an Override to Enroll the approved students into GERON401A/B. It also acts as the Department Consent Form and allows students to indicate their intended/approved usage of the course. 

Please note that you may only submit this form AFTER you have received approval from the School of Public Health Sciences (SPHS) Associate Director as well as your approved supervisor.  

This form needs to be submitted no later than one week prior to the last day to add courses.
