Request for Academic Accommodation (SAF) for Missed Assessment

As a student, you may encounter situations such as significant illnesses, ongoing medical conditions, or other circumstances that prevent you from meeting academic obligations. 

New this term (Winter 2023), there is a new process for requesting accommodation. Please see the Faculty of Arts Accommodations for Illness or Extenuating Circumstances page for direction on next steps, including submitting supporting documentation, like the Verification of Illness Form.

Ensure to contact your course instructor before or within 24 hours of a test, exam or due date and follow the instructions on the accommodations page. You should also submit documentation within 48 hours of the missed course element. Please email your instructor or safadvisor@uwaterloo.ca if you have any questions.

An accommodation will only be considered when the instructor hasn’t already indicated a policy that accommodates for an absence. For example, the course syllabus may already indicate what happens if one quiz is missed because of an absence. So check the relevant course syllabus to see if perhaps there is already some type of accommodation built into the course element itself (e.g., we take the best x of y quiz results which allows students to potentially discard a less than ideal result or two BUT also allows the instructor(s) to readily deal with a missed quiz or two without needing to resort to the process mentioned above).

Submitting an accomodation request is the first step in the process to have your situation reviewed. Submission of the form does not guarantee approval of the request.
