Final Examinations Arrangement Form

If you're enrolled only in online courses and are away from Waterloo, you can make arrangements to write your final examinations externally with a post-secondary institution test centre or approved proctor by submitting this form. Be sure to review the information on the Final examination arrangements for online courses page carefully before submitting this form. 

Note:

• It is your responsibility to make arrangements to write your examination with a post-secondary institution test centre or approved proctor, at an agreed-upon time, during the applicable examination period:
  • If your examination is scheduled on campus for December 11 or 12, you must write it externally between December 11 and 14.
  • If your examination is scheduled on campus for December 18 or 19, you must write it externally between December 18 and 2
• You are responsible for paying any associated fees to the proctor for their services, including any charges to cancel or reschedule your final examination(s). These fees will not be reimbursed by the University.

Before submitting this form, ensure your proctor meets the requirements.

This form will open on October 2, 2026, when the final examination schedule is released. This form is due on October 16, 2026.
