Final Examinations Arrangement Form

If you're enrolled only in online courses and are away from Waterloo, you can make arrangements to write your final examinations externally with an approved proctor by submitting this form. Be sure to review the information on the Final examination arrangements for online courses page carefully before submitting this form. 

Note:

• It is your responsibility to make arrangements to write your examination with an approved proctor, between April 10 and 13 at an agreed-upon time.
• You are responsible for paying any associated fees to the proctor for their services, including any charges to cancel or reschedule your final examination(s). These fees will not be reimbursed by the University.
