This web form is for new students who have not yet recieved their physical WatCard, who are currently registered and will be on campus to pick it up. Students beginning a new program who have already been issued a WatCard should bring their current card in to our service counter to be updated.
Before applying, verify that your preferred name is correct in WatIAM. Failure to complete this step, will result in a $20 reprint fee, to update the name on your card.
You must have access to your Office 365 license and your UWaterloo email in order receive the email generated from this form with the photo upload link so please make sure you do prior to submitting this form.
