HLTH472 Independent Studies enrollment form

This page contains the HLTH472 Independent Studies Enrollment Form. The form will be used as an Override to Enroll the approved students into HLTH472. It also acts as the Department Consent Form and allows students to indicate their intended/approved usage of the course. Please note that you may only submit this form AFTER you have received approval from your supervisor.  

This form needs to be submitted no later than one week prior to the last day to add courses.
