Transcript requirements

Official transcripts are required for all post-secondary schools you've attended.

Your application to the Optometry Program will require several official university transcripts to be forwarded to the Optometry Admissions Office:

  • An interim transcript is due at the same time as your Admission Information Form (AIF).  If your fall term courses don't appear on your transcript at the time you have them sent, you must have another official transcript sent by the October deadline.
  • If granted an invitation to the Meet & Greet, a transcript of your fall (September to December) course work. 
    • If you haven't taken courses that term, then a new transcript isn't required.
  • If admitted to the program, a transcript of your winter (January to April) course work. 
    • If you haven't taken courses that term, then a new transcript isn't required.
  • If you have finished all course work at the time of your initial application, then only the one transcript (due at the application deadline) is required.
  • All prerequisite courses must be completed by the May deadline before entry in to the Optometry program.  
  • Courses completed between the end of May and the beginning of September of the year you enter Optometry will not be considered by the Admissions Committee.
    • This also applies to the six terms with five courses. 
    • Applicants who have not completed all the prerequisites by the end of May deadline will not be admitted. There are no exceptions.

High school transcripts are not required except: 

  • AP and IB transcripts: if you have taken AP and/or IB courses, you must arrange for AP and IB transcripts to be sent to the University of Waterloo (see the address below).

Transcripts while attending an Ontario university

If you attended the University of Waterloo for your prerequisites, you do not need to request transcripts.

  • If you attended the University of Waterloo for your prerequisites, you do not need to request transcripts.

  • If you will be/are attending a university in Ontario (other than Waterloo), you must use the online system to request an official transcript to be sent immediately. There is a fee for this service.

  • If you will be registered in the fall term, we need to see the courses you are taking, so we must receive that transcript by the October deadline.  

  • If you are invited to the Meet & Greet, you must then contact the universities you’ve attended to request that an official transcript be mailed directly from the university to the Registrar's Office at the University of Waterloo with your end of term final fall marks.

  • If admitted, you must then contact your university to request an official transcript be mailed directly from the university to the Registrar's Office at the University of Waterloo with your end of term final winter marks. 

  • Many universities are now sending transcripts electronically. Official transcripts should be sent to myapplication@uwaterloo.ca

Transcripts from outside Ontario

If you are attending a university outside Ontario, you will have to make independent arrangements to have your official transcripts sent to the Registrar's Office at the University of Waterloo. This is typically arranged through your own university's Registrar's Office. There is usually a fee for this service.
