Most forms that you will complete will require approval from your Undergraduate Advisor and should be submitted to the Undergraduate Coordinator either in person to E5 6126, by fax 519-746-4791 or by email to sydeusec@engmail.uwaterloo.ca (either as a completed fill-able PDF or as a scan of the form completed by hand).
