Admission Information Form

The second step in the application process is the Admission Information Form (AIF).

Approximately three weeks after your on-line submission, you will receive an email from uWaterloo outlining the next steps.  Complete the Admission Information Form (AIF) and submit it by the deadline indicated on our important dates page.  (this needs to link to the important dates page). Again, you are encouraged to submit the application and supporting documents early.

Admission Information Form (AIF)

The Admission Information Form contains sections for:

Personal ID

Your personal information has automatically been filled in for you.

Academic record

All undergraduate university courses must be listed in this section.

Autobiographic sketch

The Admissions Committee is interested in learning more about your interests and achievements outside the classroom. The autobiographical sketch provides an opportunity to describe your non-academic profile, including extracurricular activities, hobbies, work and volunteer experience, athletic involvement, community leadership, club membership, and student government etc.

Confidential assessment forms

You’ll need to arrange two confidential references (from individuals who are not your relatives) from:

  • An optometrist
  • A character reference (employer, coach, professor)

Completion of coursework

Please note: Typically, you will begin the application process before you have completed all the prerequisite courses. You are responsible for completing the remaining courses and having your transcripts submitted to the School by the deadlines.

Courses completed between the end of May and the beginning of September of the entry year to Optometry will not be considered by the Admissions Committee.

Applicants who have not completed all the prerequisites will not be admitted. There are no exceptions. There are no conditional offers of admission.
