Submit the Intent to Certify Form below to receive your Student Leadership Program (SLP) certificate. The leadership certificate is an excellent way to further develop both personally and professionally.

Due to COVID-19, physical certificates will not be available until the Student Success Office (SSO) re-opens in-person services. Students graduating in spring 2020 may request a physical certificate to be mailed to their home address and current students will be able to pick-up their certificate from the SSO when we return to campus. 

Your certificate will be signed by University of Waterloo President and Vice-Chancellor Feridun Hamdullahpur, as well as Associate Provost, Students Chris Read.

Important steps: 

  • Before attending your final SLP workshop, complete and submit the form below
  • You will receive your certificate at the end of your final SLP workshop from the facilitator

Please note:

Submission of the form via email equates to an electronic signature. Please ensure your information is accurate and not misrepresented in order to qualify for the certificate, as it will be verified by the program administrator. Feedback collected through this form will be used to make informed decisions for the program. 

If you received an e-certificate from May-December 2019, you can request a physical certificate retroactively. To request your phsyical certificate, email with your: 

  • First name
  • Last name
  • Student number

Please note that you will need to come into the Student Success Office in South Campus Hall to pick up your new certificate.
