Replacement diploma

We offer two options to order a replacement diploma if your original has been lost, damaged, or your legal name has changed:

  1. Using your University of Waterloo username and password, log in to our online e-commerce Replacement Diploma Request FormIf you are unable to log in to your account complete the Forgotten username and/or Forgotten password self-serve forms. If you are still unable to access your account connect with Service Desk to reset your password manually.
  2. If you do not have a previous University of Waterloo username and password, complete our Replacement Diploma Request Form (PDF). Submit the form by email to askthecentre@uwaterloo.ca or by mail to The Centre, University of Waterloo, Needles Hall South, 200 University Ave West, Waterloo, ON N2L 3G1. Once you have submitted your form, please call our office at 519-888-4567, ext. 42268 to provide a credit card number. Cheques made payable to the University of Waterloo are also accepted.

The original copy of your diploma must be returned to our office for a replacement to be issued. If you have lost your diploma you must complete and submit the statutory declaration with your request. The Centre offers in-person Commissioner of Oaths services free of charge for those requiring the statutory declaration.

Replacement diploma requests are typically processed within five business days, plus delivery.
