Faculty of Health Graduate Mentorship Award Nomination

Please complete this form to submit your nomination for the Faculty of Health Graduate Mentorship Award.

Before beginning the form, you will need to write and obtain the following letters, and save them as one file to upload at the end of the form. All nomination packages must include the following documents:

• A cover letter by the nominator that outlines the qualities of the proposed mentor and the reason for nomination.
• A minimum of 3 and a maximum of 7 letters of support from current or prior students or post-doctoral scholars within the faculty. Each letter must include the student’s name, level of graduate student and time-frame supervised, and current position.
• A letter of support from the Associate Chair/Director Graduate Studies (or designate) for the nomination.
