How to submit course requirements
Instructors teaching in the Fall 2025 term are to complete the Health course requirements form below between February 24 to March 12 to have their preferred teaching window and meet patterns considered for Fall 2025 course scheduling. Please direct any questions to your scheduling representative.
Instructors must consider ALL of their teaching and course requirements (i.e., teaching window, constraints, meet pattern, and equipment needs) carefully before submitting this form as this is the basis for which your course will be scheduled pending chair/director approval.
The form must be submitted by the deadline in order for an instructor’s teaching preferences and course requirements to be considered. Although it is our goal to accommodate all requests, it may not be possible to do so. The chairs/director must ensure that instructors’ choices are spread throughout the week. If a selection cannot be accommodated, the chair/director will contact you.
Instructors DO NOT need to complete this form for Centre for Extended Learning (CEL) online courses.
One form can be submitted for up to three course offerings.
NOTE: Instructors requiring additional classroom space for breakout rooms, presentations, or guest speakers, must consult with their scheduling representative prior to submitting this form.
