To fill out a Change of Name form, you must provide a suitable legal document such as one of the examples listed, or provide an official statutory declaration (PDF) form that justifies your change of name.
The Centre offers Commissioner of Oaths services free of charge to University students, staff, faculty, and alumni, even if it does not relate to University business.
Updating University of Waterloo Records
Please note: changing the name on your student record means that we, as a University, report this name to provincial/external organizations. This can create potential issues with OSAP, tax receipts, health insurance, and grad school applications. This is especially important for international students, as it may impact travel visas.

This will update: Quest, official documents (transcripts, diplomas), LEARN, WatCard, WaterlooWorks, and your email.
I have not legally changed my name
...but I want to change my name in all of these systems: Quest, official documents (transcripts, diplomas), LEARN, WatCard, WaterlooWorks, and your email.
- You can update your primary name on your student record using the Change of Name form; bring the form to The Centre (NH, first floor) to initiate this change.
- You will need to complete the second page of the Change of Name form, known as the Statutory Declaration, and have it signed by a Commissioner of Oaths. You will need to confirm your name in the presence of the Commissioner.
- The Centre has a Commissioner of Oaths who can sign this form at no charge.
- If you have changed your gender designation and would like to update the University, please complete the Undergraduate Gender Update Form.
