CRT Coverage Request Form

If you would like UWCRT attend your event, please fully complete and submit this form. Note that we require minimum 14 days in order to process your request and arrange for trained first aid responders to be present at your event.

UWCRT coverage is provided free of charge as a Waterloo Undergraduate Student Association Service. We are happy to provide first aid coverage at your event subject to approval by the Service Coordinators and the Waterloo Undergraduate Student Association. While we can cover almost all events on campus (and in some cases, off-campus events), we recommend that our assistance only be requested for events that pose a risk of injury, sickness or involve alcohol consumption.
