1. Plan number This is your policy number (57130) for extended health and dental benefits which includes prescription drugs.
2. I.D. No. This is your six digit employee ID number with four zeros in front.
3. Version number This is the version number of your benefit card. Each time you order a new benefit card your version will change.

Card features

The benefits ID card is an electronic payment system that provides on-the-spot processing of prescription drug claims at almost any pharmacy in Canada. It’s a convenient, easy-to-use alternative to submitting claim forms.

You should receive your benefits ID card within 4 to 6 weeks after your benefits become active. For inquiries about that status of your card, lost cards or any other information relating to your benefits ID card, please contact Human Resources at extension 35935 or hrhelp@uwaterloo.ca.

How to use your card

Members and eligible dependents simply present their benefits ID card when having prescriptions filled. The pharmacist uses the card to confirm eligibility and drug coverage. The amount covered is electronically paid to the pharmacist, and the covered individual pays any portion not covered by the benefit.
