Extended health care claims

When completing paper claim forms, you will need to include the policy number and your employee identification number. The policy number for extended health care (including prescription drugs) and dental claims is 57130. Out-of-country claims are processed under 325156.

For all extended health care expense claims except prescription drugs:

  1. Complete the Extended health claim care form (PDF).
  2. Mail the completed form and original receipts to:

Prescription drug claims

Your prescription drug claims are settled right in the pharmacy with your Assure Card™.

When you purchase a prescription drug:

  • Present your Assure Cardtm to your pharmacist.
  • Your pharmacist will use the card to verify your eligibility and drug coverage, and to apply any deductibles or coverage limits.
  • You pay only for fees not covered by your plan and receive a printed explanation of benefits.
  • You do not have to fill out any claim forms or wait for reimbursement. It's all handled on the spot.
  • Pharmacies that recognize the Assure CardTM are also linked electronically with the provincial drug benefit plans. The pharmacist will know how much your provincial plan will cover, and how much you can claim under your health plan.

Using paper claim forms

The Assure CardTM is recognized by more than 7,500 pharmacies nationwide.

If you can't use your Assure CardTM:

  1. Complete the Extended health claim care form (PDF).
  2. Mail the completed form and original receipts to:

Dental claims

Depending on your dentist, you may need to pay up front and be reimbursed or you may need to pay only the portion not covered by the benefit. If you have to to pay up front and your dentist does not submit the claim electronically, you can complete a paper claim from to submit to Great-West Life:

  1. Complete a Dental claim form (PDF).

Important claiming information

It is important that all forms be completed in full, including the appropriate identification number and payment receipts, to avoid delays in processing claims. We recommend you keep a copy of all items submitted.

All claims must be submitted to the carrier within 12 months from the end of the calendar year in which the expenses were incurred to be eligible for reimbursement. For example, all eligible expenses incurred in 2015 must be submitted by the end of 2016.

Claim forms must be completed with group number, certificate number and signature before submission, otherwise they will be returned by the insurance carrier thereby delaying the reimbursement process.

On any occasion when a significant expense is expected, employees are encouraged to submit a cost estimate or predetermination/treatment plan to Great-West Life beforehand. This can be done by having the practitioner provide a predetermination of benefits. Procedure coverage and reimbursement amounts will be communicated by our insurer in writing to the employee.
