AUO Advisor Appointment Request Form

The Arts Undergraduate Office advising team provides resources and supports for students in the Bachelor of Arts (BA) degree program.

There are separate advising offices for AFM, CFM, SFM and GBDA students.  

Advising appointments are booked Monday to Friday, 10-11:30 am and 1-3:30 pm.
Please provide availability only within these times. 

The AUO offers in-person, or virtual appointments through Microsoft Teams, during regular advising hours. 

  • If you are co-registered with St. Jerome's University (SJU): Please use the SJU appointment booking form.  
  • If you are co-registered with Renison University College (REN), visit Renison Academic Advising. 
  • If you are an upper-year student with a declared major, contact your major advisor first for questions related to your major/other academic plan – see the list of  Arts Major Advisors.

Appointments are booked Monday-Friday between 10:00-11:30 am and 1:00-3:30 pm.
Please provide a few options for your availability, so we can book you in as soon as possible if your preferred date and time isn't available.

Consent
If you are planning to have a support person (parent, UW MATES, friend, etc.) join your appointment, please let us know so that we can provide a Consent to Release Personal Information form. This form allows you to control the personal information (grades, academic standing, etc.) shared by your advisor during your appointment.
