You are invited to participate in a research study conducted by Lori Case (Associate Dean, Co-operative Education, Faculty of Mathematics) and Jeremy Steffler (Faculty Relations Manager, Math, Co-operative Education) of the University of Waterloo, Canada. The Faculty of Mathematics, in consultation with a number of on-campus stakeholders, has prepared a preliminary draft of Learning Outcomes for Co-operative Education Degree Programs. The objectives of the research study are to obtain feedback on these outcomes.

We hope that you will take this opportunity to review the draft outcomes document and take ~10 minutes to share your perspective with us. You will be asked demographics questions, as well as questions about your educational experiences. Participation in this study is voluntary. You may decline to answer any questions that you do not wish to answer, and you can withdraw your participation at any time by not submitting your responses.

When: July 19, 2021 to September 30, 2021

Why:
 A Working Group of the Faculty of Mathematics’ Undergraduate Affairs Committee (referred to as the “Working Group” for purposes of this survey) developed draft learning outcomes for the Faculty's co-operative education (CE) degree programs.

The majority of Faculty of Mathematics majors can be pursued as CE or non-CE (regular) degrees. Students pursuing CE degrees have additional degree requirements (e.g., completion of work terms, work term reports, professional development courses) that are not required for regular degrees. The Faculty is attempting to identify the learning outcomes specifically associated with these additional degree requirements (i.e., what additional learning outcomes are anticipated for CE students).

Having a clear understanding of the learning objectives for CE programs will help guide the Faculty as it reviews its academic policies and degree requirements associated with CE over the coming years.

Survey participation: Participation in this study is voluntary. You may decline to answer any questions that you do not wish to answer, and you can withdraw your participation at any time by not submitting your responses.

How results will be used: The University will use the results of the survey to improve the quality of program and services at the University of Waterloo for current and future students. Public reports, including web pages and scholarly articles, will include only summarized results, ensuring that no individual student can be identified. Only members of the Research Team will have access to the data.

Privacy: Your identity will be confidential. The survey is anonymous, all of the data will be summarized, and no individual could be identified from these summarized results. Furthermore, the website is programmed to collect responses alone and will not collect any information that could potentially identify you (such as machine identifiers). Please Note: We do not collect or use internet protocol (IP) addresses or other information which could link your participation to your computer or electronic device.

Because this is an anonymous survey the researchers have no way of identifying you or getting in touch with you should you choose to tell us something about yourself or your life experiences. Once submitted, the data cannot be withdrawn as the researchers will have no way of knowing which data was collected from which participant.  

The data, with no personal identifiers, collected from this study will be maintained on a password-protected computer database maintained by the university. As well, the data will be electronically archived after completion of the study and maintained for at least 1 year.

You will be completing the study via an online survey operated by Qualtrics. Qualtrics has implemented technical, administrative, and physical safeguards to protect the information provided via the Services from loss, misuse, and unauthorized access, disclosure, alteration, or destruction. However, no internet transmission is ever fully secure or error free.

By choosing to participate in the survey you agree that you have read and understood the privacy terms of doing so. The Faculty of Mathematics appreciates your decision to participate.

Risks: There are no known or anticipated risks from participating in this study.

By agreeing to participate in the study you are not waiving your legal rights or releasing the investigator(s) or involved institution(s) from their legal and professional responsibilities.

By completing the survey, you agree to allow your data to be used for research purposes.
